List of documents for a social scholarship

To write an application for a social scholarship, you need to have the following list of documents, according to the category you fit, namely:

  1. Orphans and children deprived of parental care, and persons among them in the case of continuing education until the age of 23 or before graduation, as well as students who lost their parents during the period of study between the ages of 18 and 23

List of documents:

  • Passport (and extract if ID)
  • Identification code
  • Student ticket (or certificate from the place of study)
  • a copy of the decision of the guardianship authority on granting the status of an orphan or a child deprived of parental care
  •  extract from the registration and statistical card of an orphan and a child deprived of parental care
  • a copy of the death certificate of the parents.
  1. Persons entitled to receive a social scholarship in accordance with Articles 20-22 and 30 of the Law of Ukraine “On the Status and Social Protection of Citizens Affected by the Chornobyl Accident”;

List of documents:

  • Passport (and extract if ID)
  • Identification code
  • Student ticket (or certificate from the place of study)
  • a copy of the relevant certificate confirming the status of the person affected by the Chernobyl disaster
  • St. about birth.
  1. Miners who have at least three years of underground work experience in accordance with Article 5 of the Law of Ukraine “On Increasing the Prestige of Mining Work”;

 List of documents:

  • Passport (and extract if ID)
  • Identification code
  • Student ticket (or certificate from the place of study)
  • certificate from the mining enterprise indicating the experience of underground work.
  1. Persons who became students within three years after obtaining basic and / or complete general secondary education, whose parents are miners who have at least 15 years of underground work experience or died as a result of an accident at work or who have a disability of group I or II, in accordance with Article 5 of the Law of Ukraine "On increasing the prestige of mining";

  • Passport (and extract if ID)

  • Student ticket (or certificate from the place of study)
  • certificate from the mining enterprise indicating the experience of underground work;
  • St. about birth.
  1. Persons recognized as victims of the Revolution of Dignity, participants in hostilities in accordance with the Law of Ukraine "On the status of war veterans, guarantees of their social protection" and their children (until graduation, but not longer than 23 years)

List of documents:

  • Passport (and extract if ID)
  • Identification code
  • Student ticket (or certificate from the place of study)
  • a copy of the certificate of the injured participant of the Revolution of Dignity of the established sample

or

  •  a copy of the certificate of the participant of hostilities of the established sample
  • certificate of the social protection body on registration in the Unified State Automated Register of Persons Eligible for Benefits at the registered (actual) place of residence with reference to the legislation according to which the status of a participant in hostilities is established, or a copy of the certificate of direct participation persons in the anti-terrorist operation, ensuring its implementation and protection of independence, sovereignty and territorial integrity of Ukraine, or a copy of the certificate of participation in the implementation of measures to ensure national security and defense, repel and deter armed aggression in Donetsk and Lugansk regions, ensure their implementation .
  1. Persons whose parents died (went missing) in the area of the anti-terrorist operation, measures to ensure national security and defense, repel and deter armed aggression of the Russian Federation in Donetsk and Luhansk regions, hostilities or armed conflicts or died of injuries, contusions injuries received in the area of the anti-terrorist operation, measures to ensure national security and defense, repulse and deter armed aggression of the Russian Federation in Donetsk and Luhansk regions, hostilities or armed conflicts, as well as due to illness received during the anti-terrorist operation , implementation of measures to ensure national security and defense, repulse and deter armed aggression of the Russian Federation in Donetsk and Luhansk regions (until graduation, but not longer than until they reach 23 years of age);

  • a copy of the death certificate of the father (mother);
  • a document confirming the death (disappearance) of one of the parents, issued by a military formation, a law enforcement agency of special purpose, or a court decision declaring him missing (declaring him dead);
  • a copy of the certificate of medical and social examination.
  1. Persons whose parents died during mass civil protests or died as a result of injury, contusion or mutilation received during mass civil protests (before graduation, but not longer than 23 years of age);

  • Student ticket (or certificate from the place of study)
  • a copy of the death certificate of the father (mother);
  • a copy of the certificate or certificate of the established sample of the family member of the deceased (deceased) war veteran;
  • certificate of the social protection body on registration in the Unified State Automated Register of Persons Eligible for Privileges at the registered (actual) place of residence with reference to the law, according to which the status of a family member of a deceased (deceased) war veteran is established .
  1. Persons registered as internally displaced persons (before graduation, but not longer than 23 years of age)

List of documents:

  • Passport (and extract if ID)
  • Identification code
  • Student ticket (or certificate from the place of study)
  • a copy of the certificate of registration of an internally displaced person.
  1. Persons who have been awarded social scholarships of the Verkhovna Rada of Ukraine for students from orphans and children from low-income families in accordance with the resolution of the Verkhovna Rada of Ukraine of October 24, 2002 № 218-IV

List of documents:

  • Passport (and extract if ID)
  • Identification code
  • Student ticket (or certificate from the place of study)
  • a copy of the order of the Cabinet of Ministers of Ukraine on the appointment of social scholarships of the Verkhovna Rada of Ukraine to students of vocational higher and higher education institutions from among orphans and children from low-income families.
  1.  Children with disabilities and persons with disabilities of I-III groups

List of documents:

  • Passport (and extract if ID)
  • Identification code
  • Student ticket (or certificate from the place of study)
  • a copy of the medical report on a disabled child under the age of 18 or a copy of the certificate of medical and social examination;
  1.  Students from families receiving assistance in accordance with the Law of Ukraine "On State Social Assistance to Low-Income Families";

  • Passport (and extract if ID)
  • Identification code
  • Student ticket (or certificate from the place of study)
  • certificate of the body of social protection of the population on the appointment of family assistance in accordance with the Law of Ukraine "On state social assistance to low-income families".
  1.  Persons with disabilities as a result of war in accordance with the Law of Ukraine “On the Status of War Veterans, Guarantees of Their Social Protection” and Their Children (until the children finish school, but not longer than until they reach 23 years of age)

  • Passport (and extract if ID)
  • Identification code
  • Student ticket (or certificate from the place of study)
  • a copy of the identity card of a person with a disability as a result of the established war.
  1.  Persons living in settlements on the line of contact (before graduation, but not longer than until they reach 23 years of age).

  • Passport (and extract if ID)
  • Identification code
  • Student ticket (or certificate from the place of study)
  • a certificate of registration of the place of residence of the person of the established sample, which contains information on the registration of the place of residence of the person in the settlement on the line of contact.
